(A) Instructional
Permit Requirements:
1.Be at least 15 ½ years old.
2. Have finished Driver Education (in-class
instruction), and be enrolled in Driver's Training (behind the wheel)
at the same time.
3. Driver's Training must be started with
a licensed driving instructor before using the insructional permit.
4. Pass a written test on traffic laws (46
questions; and answer 38 correctly), written test failure will result
in a one week waiting period before the exam may be taken again.
5. Must pass eye examination. 6. May use the instructional
permit to practice with a parent, guardian, spouse, or other adult
age 25 or older who has a valid Drivers License, or is a licensed
driving instructor.
(B) In order to obtain an instruction permit,
students should bring to the DMV the following:
1. A completed Parental Consent Form (DL-44),
including signatures. $27.00, either cash or check made payable
to DMV.
2. Certificate of completion of classroom
driver education (Blue OL-237) and Certificate of enrollment in
driver training (White OL-392)
3. One (1) of the following:
(a) Original or certified copy of a County
Birth Certificate
(b) State ID Card
(c) Current Passport.
4. Glasses or contact lenses.
5. Proof of Social Security Number, i.e. Social
Security Card, Pay check stub, Military Id Card
(C) Provisional License Requirements:
1. Be at least 16 years old.
2. Have an instructional permit for at least
6 months.
3. Proof that you have completed Driver's
Education and Driver's Training.
4. Birth Certificate, (or other items, listed
in section B).
5. Complete 50 hours of supervised driving
practice (10 hours must be night driving). Your parents must certify
to this training.
6. Pass the behind the wheel driving test.
If you fail the test you must wait two weeks before you are tested
again. You have three chances to pass the driving test while your
permit is valid. A car that's in safe driving condition, with proof
of insurance.
